Where are you located?
We are located at 21301 Powerline Road, Suite 203, Boca Raton, Florida 33433. Our office is on the second floor, toward the back of the building.

Do you offer telehealth?
Yes. We offer telehealth appointments to patients located in Florida, New York, Utah, Iowa, and Colorado.
What insurance do you accept?
We currently accept Florida Blue commercial plans, Medicare, and select Medicare Advantage plans, with more insurance plans coming soon. We also welcome self-pay patients and are a preferred provider with United Refuah HealthShare. If you belong to another health-sharing plan, contact us to see if we can work with it.
How do I become a primary care patient?
You can join our direct primary care membership for $150 per month for an individual or $350 per month for a family. Membership gives you an ongoing relationship with your primary care provider, and we care for patients of all ages. Prefer to use insurance? You can also see us through an accepted insurance plan or United Refuah HealthShare without joining the membership.
